Horse(s) Owner hereby voluntary releases and forever discharges the Transporter and its agent or employees from any and all liabilities, claims, demands, actions or rights of actions, which are related to, rise out of, or are in any way connected with the transport of the aforementioned horse(s) including the omissions of the Transporter, its agents or employees, and all other persons or entities, for any and all injuries, illness or disease, and damage to the aforementioned horse(s) including all third party claims. While every effort is to be made to maintain a safe environment and safe travel conditions for each horse in our care, and while we do not anticipate any problems, if accidents do occur, and therefore Premier Horse Transport, its owner agents, affiliates, employees, representatives will not be held responsible for any personal and/or property damage that may occur during loading, transport and unloading of the aforementioned horse. Owner further agrees, promises and covenants not to sue, assert or otherwise maintain or assert claims against the Transporter or its agents or employees, for any injury, death, disease, or damage to the aforementioned horse(s) arising from or in connection with the transport, care or feeding of the said horse(s) or from any claims asserted by other third parties. Owner agrees that he/she assume expenses or liabilities associated with the transport or care of the aforementioned horse(s). Owner hereby holds harmless the owner of Premier Horse Transport from any liability that would incur for property damage or bodily injury to any horse(s) and/or owner/passengers loading, transporting, unloading, or handling the aforementioned horse(s).

 

Authority: I authorize the transportation of my horse for the cost indicated in the form above.  I understand that all professional care will be taken to provide for the safety of my horse(s) and authorize veterinarian care at my expense should it be necessary.

 

This Entire Agreement: Owner/Agent understands that this is the entire agreement between the Owner/Agent and the Transporter, its agents or employees. It supersedes and cannot be modified or changed in any way by the representations or statements of any employee or agent or the Transporter or Owner/Agent.

 

BY CHECKING THE “I AGREE” BOX INDICATES THAT I HAVE READ THIS ENTIRE DOCUMENT, UNDERSTAND IT COMPLETELY AND AGREE TO BE BOUND BY ITS TERMS IN ITS ENTIRETY.

 

BY CHECKING THE “I AGREE” BOX YOU CERTIFY THAT YOU ARE WHO YOU CLAIM YOU ARE.  ALSO, YOU ARE THE OWNER OR THE OWNER'S AGENT OF THE HORSE(S) AND THE PERSON WHO IS CONTARCTING WITH US.  YOU ALSO AGREE, THAT BY CHECKING THE “I AGREE” BOX, IT CONSTITUE AS YOUR ELECTRONIC SIGNITURE, WHICH MAKES YOU BOUND BY THIS ENTIRE AGREEMENT.
